What is the probability of an explosive failure (with major oil spill and or fire) due to a transformer internal arcing fault?

The average probability of an explosive failure over a transformer’s lifetime of 40 years is 40%, where 4% of it is leading to a catastrophic oil fire and 22% to a major oil spill. Therefore a catastrophic failure of a transformer is not a negligible risk.
